ESCCI Seeks Proposals for Services Enabling Persons Age 60 and Older to Remain Living in their Homes and Communities

REQUEST FOR PROPOSALS

Elder Services of Cape Cod and the Islands, Inc. is soliciting proposals for the Title III B & D funds received from the Older Americans Act of 1965, as amended, for the delivery of services to elders and caregivers in Barnstable, Dukes, and Nantucket Counties (with the exception of Gosnold). Grants will be awarded through a competitive proposal review process, to support and extend comprehensive and coordinated services enabling persons age 60 and over and/or their caregivers, to remain living in their homes and communities.  Proposals must support or extend (1) access to services, (2) in-home services, and/or (3) legal services.  In addition, separate funding is available for Title IIID evidence-based programs which promote good health and well-being.  Individual grant awards not to exceed $10,000 (except for the legal services grant).

Special emphasis must be given to rural elderly, elders with the greatest economic need, low-income minority individuals, disabled and limited English speaking elders, LGBT elders and people with Alzheimer’s or other dementia related illnesses and their caregivers.

Additional priorities for Title III B proposals for services include but are not limited to:

  • Support services for Caregivers, including respite
  • Technology education and assistance
  • Transportation and transportation assistance (i.e., driver escort or bus monitor “to and through” the door)
  • Supports for elders who are deaf, hard of hearing, blind or experience low vision
  • Home modification
  • LGBT supportive services
  • Services which reduce isolation
  • Access to information about community services through outreach
  • Other innovative programs

Grant year is October 1, 2023, through September 30, 2024, with the option (based on performance and funding) to roll over grants for one additional year.

“Letters of Intent” are requested from interested parties and should be submitted with contact information, a brief program description, budget estimates, number of unduplicated elders / caregivers to be served, estimated service units, and community or communities to be served. The deadline for receipt of Letters of Intent is Friday May 26th, 2023 @ noon. Grant applications will be emailed to those who submit a Letter of Intent.  Attendance at RFP Information Session is encouraged (Wednesday 5/17/23 at 10:00am via Zoom.  Contact Sarah Franey for Zoom Link or call-in phone number).
