Meals-on-Wheels Now Offers Medically-Tailored Options

This fall, Meals-on-Wheels began introducing medically-tailored menu options for residents of Barnstable County. If you are living with a chronic condition such as high blood pressure, diabetes or kidney disease, these meals could help you better manage your symptoms. Talk to your ESCCI care manager or meals site coordinator for more information. Sign up for Meals-on-Wheels using this online referral form.

CARDIAC

The Cardiac menu may also be referred to as “Heart Healthy” or “Low-Sodium.” It helps control blood pressure and cholesterol, reducing the risk of a heart attack or stroke.

Cardiac meals limit sodium (< 500mg), total fat (< 20 grams), and saturated fat (< 6 grams).

Total milligrams (“mg”) of sodium for the meal and for each item in the meal is provided on the cardiac menu.

CARB-CONTROLLED

The Carbohydrate (“Carb”)-Controlled menu may also be referred to as “Consistent Carb.” For people with diabetes or at risk of diabetes, it helps manage blood sugar levels.

Carb-Controlled meals are also “heart-healthy” by limiting sodium (< 600mg), total fat (< 20 grams), and saturated fat (< 6 grams). Additionally, they restrict carbs to 60-75 grams.

Total grams (“g”) of carbs for the meal and for each item in the meal is provided on the carb-controlled menu. Anyone who wants to limit carbs further at this meal can refer to these numbers to decide if one or more items should be saved for another meal or snack.

RENAL – starting 12/1/26

The Renal menu may also be referred to as “Kidney Friendly.” Your kidneys help keep nutrients balanced in your body and remove waste products from your blood. If you have kidney disease, some nutrients can build up and damage your kidney, causing further kidney failure.

Renal meals limit sodium (< 560mg), phosphorus (< 300mg), and potassium (< 800mg). They contain a moderate amount of protein (30 to 40 grams).

Total milligrams (“mg”) of sodium for the meal and for each item in the meal is provided on the renal menu.
