Elder Services of Cape Cod and the Islands collaborates with the Visiting Nurse Association in Healthy Living Cape Cod to provide Tier 1, Evidence-Based Programs to the community at no cost. Evidence- Based Programs are supported by research to help promote health and prevent disease. Here’s information on upcoming programs for Spring 2023. Fit & Strong! is an evidence-based program that combines flexibility, strength training, and aerobic walking with health education for sustained behavior change among older adults with lower extremity osteoarthritis. Each class will begin with an hour of exercise and end with a 30 minute health topic discussion. Offered at the Harwich Council on Aging on Wednesdays and Fridays from 1:30 – 3:00 p.m. Class begins on March 3, 2023 and ends on June 9, 2023.

Chronic Disease Self-Management Program is for adults experiencing chronic health conditions such as: hypertension, arthritis, heart disease, stroke, lung disease, and diabetes. The overall goal is to empower you to assume an active role in maintaining your health and managing your health conditions. Offered at the Brewster Council on Aging, Fridays from 9 -11:30 a.m. Class begins on April 7, 2023 and ends on May 12, 2023.
